Trainees are required to register (or “re-register”) with the University of Toronto AND each hospital where they rotate, each academic year (July 1st).

Welcome to the Wightman-Berris Academy!

You are required to begin your 2015-16 registration at UHN ONLY or both UHN AND MSH using our new online registration system UMLearns. If you will be at both UHN and MSH for rotations during the academic year, or will be cross-covering both sites, you must register for both places. After registering online you must present in person to UHN (and MSH if registering for both) to complete the process. If you have a photo ID from us already, please bring it with you for re-activation.

Please ensure that you are registered with the University of Toronto FIRST, before you register on line with UMLearns.

PHOTO IDENTIFICATION/BADGE: Once registered, take red form to Human resources at600 University Avenue, Main Floor Room 301.  You must wear photo identification at all times in the hospital

SECURITY ACCESS CARD:  Take current photo ID badge along with red form to Security on the Main Floor, 600 University for a new Security Access card, or to update current card.

PAGERS & SCRUB SUIT DISTRIBUTION & COLLECTION:  One $80 refundable deposit will be collected for both a pager and/or 2 pairs of scrub suits.

COMPUTER TRAINING – POWERCHART ON-LINE PATIENT RESULTS, ORDER ENTRY & EMEDS TRAINING:   Please note training is mandatory. 

Part 1 of the training can be done from any high speed internet (www.mtsinai..ca/tbt_online.).  In the username field: type: tbtonline and in the password field type: training.   Part 2:  Drop in to complete your practice session in Systems Education 19th floor, room 19-226, Murray St. Elevators. Times: Mon, &, Thurs 9 am.-11am. &

1 pm – 2 pm, Tues & Fri 9 am – 11 am , Closed on Wednesday.  Upon completion of training you will be issued a password.  This password will be valid from the date of issue until June 30, 2013

You will receive a health and Safety handbook and must sign off on knowledge of related procedures

MASK FIT TESTING on a mask stocked at Mount Sinai Hospital is mandatory for your placement.  If you have you have been mask fit tested elsewhere please bring proof of the results of your fit testing with you

HEALTH RECORDS: (HRS) is open 24/7. Room 460 (Transcription Centre) to request access to the dictation/transcription system.  Please note: Without your dictation number, you will be unable to dictate patient records.  Hours of operation for the Transcription Centre, Health Records for assigning Dictation I.D. access codes are from Monday – Friday from 7:30-15:30.  Holidays and weekends the Transcription Centre is closed.

HOUSESTAFF  LOUNGE:  18TH Floor, entry code: contact Medical Education

LOCKER ROOMS:  Room 136 (Level 1) for Entry code contact Medical Education.   Choose a locker and do not forget to mark your locker number in notebook attached to the side of locker room door.  Empty the locker and remove the lock at the end of your hospital rotation.

POWER

Before registering at any hospital, trainees must complete the registration process with the University of Toronto. This is completed on the POWER website.

Hospital Registration

Once registration with the University of Toronto is completed, trainees must print a copy of their Confirmation of Registration (See below) and present this document at the Medical Education office of the hospital where they are rotating.

  • Note: Both the University Health Network and Mount Sinai Hospital will require a copy of the Confirmation of Registration to attach to the registration form. Please print enough copies of your Confirmation of Registration.

Trainees are required to fill out a registration form and any additional forms needed to complete the registration process at the hospital.

Trainees should be prepared to present a copy of their Mask Fit Testing Certification when registering.

Powerchart

Medical Trainees must complete their clinical application education before they are granted access to the PowerChart / Firstnet Electronic Patient Record. This education consists of a series of online eLearning courses followed by mandatory in-class scenario exercises. LMS eLearning Instructions-Physicians

Photo Identification

Once registered with the Medical Education Office, trainees will be given a form to take to Human Resources for a Photo ID Badge.

Occupational Health and Safety

Any workplace injury, contact with or ill health from communicable diseases must be reported to the Occupational Health and Safety Department immediately. After hours or on stat holidays, Housestaff requiring immediate medical attention should go to the Emergency Department. Mask Fit Testing

All trainees must be fitted for a respiratory mask. The fitting must be repeated every two years. Trainees must present proof of completed mask fit testing upon registration.

Please inquire at the Medical Education offices for more information, and the dates and times when Mask Fit Testing occurs.

Uniform Control

Trainees are required to wear scrubs unique to the hospital where they are rotating. Please ensure you have the appropriate scrub sets.

Commissioner of Oaths

Before contacting the Commissioner of Oaths at UHN orMount SinaiHospital, please ensure that a Commissioner of Oaths is acceptable to verify your documents. If you require a Notary Public, please contact the Medical Affairs office at the appropriate hospital.

The Commissioner of Oaths at Mount Sinai Hospital is Dr. Maureen Shandling (Senior Vice-President, Medical). Dr. Shandling can only verify documents as they relate to hospital business. There is no charge for this service. NOTE Residents must be rotating atMount SinaiHospital in order to receive this service.

Requirements:

  • You MUST present a valid Mount Sinai Hospital Photo ID Badge.
  • Documents must be filled out, but not signed (this must be done in the presence of the Commissioner and all photos and copies must be attached).

Housestaff Lounge

The Housestaff Lounge atMount SinaiHospitalis located in the main hospital building. Check with the Medical Education office for the access code.

Library Services

The Sidney Liswood Libraryis located on the 18th floor. TheLibrary is an excellent resource for Mount Sinai Hospital Housestaff, to support the hospital’s commitment to excellence in patient care, education and the advancement of research. The staff is available to provide information services to all staff. Other services include borrowing, remote access, interlibrary loans, E-Resources, photocopying and printing.

The Sidney Liswood Library at Mount SinaiHospitalis open from Monday to Friday, from 8:30 am to 6 pm. The Library is closed on weekends and statutory holidays.

Lockers

There are lockers available for Mount Sinai Housestaff. Check with the Medical Education Office for more information.

Observerships

Observerships are not organized through the Medical Education offices at either UHN or Mount Sinai Hospital.  Individuals wishing to participate in an observership should contact the department hospital directly.

Pagers

Travel Reimbursements

MSH Travel Reimbursement Form

In accordance with the 2008 PAIRO Collective Agreement, as stated in Article 19.7 (b), “the Hospital will pay taxi charges to a maximum of $70.00 per month on presentation or appropriate receipts.”

This is in circumstances where:

  • a trainee is on Out of Hospital Call and can respond within the hospital’s Medical Advisory Committee (MAC) approved rules and regulations regarding specified response time,
  • the trainee does not have a parking pass, and
  • the requirement to attend for clinical duties occurs after 6 pm and before 6 am.

Travel Reimbursements for MSH must be signed by an individual (supervisor or administrative contact) who can verify the calls taken.  The form can then be submitted to:

Vacation Requests

Vacation requests are done departmentally. Contact the administrative person for the department in which you will be rotating when you wish to request vacation.

All vacation requests must be submitted at least 30 days before the proposed vacation dates and may be made at any time throughout the year, even after March 1st, EXCEPT where vacation time is being taken to write a certification exam, which may be requested at any time including within the 30-day period before the exam.

Dictation

Health Records is open 24 hours, seven days a week. Hours of operation for the Transcription Centre, health Records for assigning Dictation ID access codes from Monday to Friday, 7:30 am to 3:30 pm.
