REGISTRATION INFORMATION
JULY 1, 2018 – JUNE 30, 2019

Trainees are required to register each academic year with the University of Toronto AND each hospital where they do their rotations.

You can begin your 2018-19 registration using our online registration system UMLearns. You must register as a Medical Learner. During the academic year if you will be at both UHN and MSH for rotations,  or will be cross-covering both sites, you must register for both places. Enter your rotations and be sure to click submit. Approvals on UMLearns typically take 72 hours, however, during high intake times this may be longer. You must be registered with the University of Toronto Postgraduate Medical Education (PGME) and have a “confirmation of registration” available on POWER for your UMLearns placement to be approved.

POWER

Before completing your registration at any hospital, trainees must complete the registration process with the University of Toronto. This is completed on the POWER website. You must be fully registered on POWER and have a Confirmation of Registration before presenting in person at Medical Education.

Electronic Patient Record (EPR)

Submit a placement and create a profile as a Medical Learner on UMLearns for this academic year prior to completing EPR training. You can begin your EPR training while waiting for your placement to be approved. If you have used your EPR account within the last 2 years your account can be reactivated without additional training. The training takes 1.5-2 hours and can be completed online. Once you have successfully completed the course be sure to complete the Course Completion Declaration Form and ID Verification Form. Accurate completion of these forms is required for you to receive timely EPR access. Upon your approval on UMLearns.uhn.ca you will receive an email confirming your registration. You will now have the following available under Network Log-in tab in your UMLearns.uhn.ca profile:
– T-ID
– EPR log-in information (NOTE: your EPR credentials will not be visible until you have completed the training).

If you complete EPR training after your UMLearns placement has been approved (you will informed via email), email MedicalEducation@uhn.ca with EPR Test Completed subject line. Access will be granted ASAP.

Call Stipends

Resident Direct Input is the new online system in which all residents enter their own Call Stipends directly into POWER.  This will launch city-wide on February 1st, 2015 and will replace all paper call stipend systems currently used.  Email notices with screen shot instructions will be sent to all residents in the coming weeks.  In addition, all residents will receive 3 notices to invite and then remind them to enter their stipends electronically.  Please visit this link for additional information and FAQs – http://www.pgme.utoronto.ca/content/call-stipends-faqs

Call Rooms

At both UHN and MSH, call rooms for trainees are coordinated through individual departments.

Photo Identification

Trainees will be issued a Photo Identification Card for each hospital where they rotate. Photo ID must be worn at all times in the hospitals. Your UHN issued Photo ID also gives you security access at UHN. If you have an existing UHN Photo ID it will be reactivated upon the approval of your registration. If you do not have a UHN issued badge visit Photo ID office:

Parking

Parking is available for UHN trainees at a discounted rate of $16.00 per 24 hours. Parking Value Cards are issued through the UHN Parking Garage. Trainees must present a letter from the Medical Education Office at Toronto General Hospital as proof that you are eligible for discounted parking. There is a refundable deposit required.

Please note: Parking Cards are in high demand and short supply. Trainees will only be issued parking cards for CLINICAL APPOINTMENTS at Toronto General Hospital, Toronto Western Hospital or Princess Margaret Hospital.

UHN Parking Cards are not valid for rotations at Mount Sinai Hospital, or for trainees taking call or with research appointments. Parking Cards must be returned to the UHN Parking Garage within 7 days of the last day of the rotation.

Occupational Health and Safety

The Occupational Health & Safety Department is available to help Housestaff with any work-related injuries or safety concerns.

MACK Foundation UHN – Postgraduate Trainee Bursary

To Support Resident Travel for Research and/or Education Meeting

UHN PGY’s 1, 2, and 3 who have been invited to present a poster or give an oral session for a major clinical, scientific or educational meeting outside of Toronto are eligible to apply for a Travel Stipend covering up to $2500.00.

Four Stipends will be awarded annually — two in September and two in January of each year.

To be eligible you must have no other available funding.  You must provide the abstract submitted and letter of acceptance.

If selected, in order to claim your funds you must submit in full conference fee, flight/transportation receipt, boarding pass, and hotel accommodation receipts.

Mask Fit Testing

All trainees must be fitted for a respiratory mask. The fitting must be repeated every two years.  For mask fit testing please visit the Occupational Health office at our site:

Notary Service

Notary service available Tuesdays between 1:00pm and 3:00pm. The cost of certification/notary of original documents is $20.00 per document. The cost for vertification/notary of copies of additional original documents is $10.00 per copy. Please present UHN identification badge at the time of notary.

Notarization of Signature

  • Please bring identification with picture and signature (ie Ontario Driver’s License, passport)
  • Document must be signed in Notary’s presence

For Certified Copies of Documents:

Uniform Control

For a set of UHN Scrubs please print and complete a Uniform Control Form. Bring the completed Uniform Control Form to the Cashier’s Office to pay the deposit then visit Linen Services to pick up your scrubs. The deposit is refundable once scrubs or lab coats are returned at the end of the clinical placement.

The deposits required for uniforms at the University Health Network are as follows:

  • $25 for 1 scrub set
  • $30 for 1 lab coat
  • $55 for 1 lab coat and 1 scrub set
  • $80 for 1 lab coat and 2 scrub sets

Shuttle Bus

There is daily shuttle bus service between Toronto General Hospital and Toronto Western Hospital. Information for UHN Shuttle Bus Service

Note: There is no service on weekends and statutory holidays.

Commissioner of Oaths

Before contacting the Commissioner of Oaths at UHN or Mount Sinai Hospital, please ensure that a Commissioner of Oaths is acceptable to verify your documents. If you require a Notary Public, please contact the Medical Affairs office at the appropriate hospital.

The Commissioner of Oaths at University Health Network is Anne Marie Holmes (Manager, Medical Education).

Ms. Holmes can only verify documents as they relate to hospital business. The charge for this service is $25 per signature.

Requirements:

  • You MUST present a valid University Health Network Photo ID Badge.
  • Documents must be filled out, but not signed (this must be done in the presence of the Commissioner and all photos and copies must be attached).

Housestaff Lounges

There is a Housestaff Lounge for trainees located at Toronto General Hospital, Ground Floor, Norman Urquart Wing, Rm. 123. The punch code for the door is available from the Medical Education Office.

Library Services

University Health Network Libraries are located at all four hospital sites. Both staff and resources are available to provide the most current research and information services to physicians, staff and trainees. Information Specialists are available at all sites to assist with any questions about services or library collections. Services available to library patrons includes: information tools, computer lab, literature searches, database/virtual library training, document delivery, photocopying and printing.

Lockers

The Medical Education department has a limited number of lockers for trainees. Check with the Medical Education office at Toronto General Hospital about the availability of lockers. If no lockers are available, check with the department where you are rotating.

Pagers

Pagers are issued through the Medical Education Offices. Trainees based at one hospital for an entire academic year will be issued a pager for the duration of the year. Trainees who do not have base hospitals will be required to return their pagers at the end of their rotation.

Residents who require a pager must obtain a Pager Form from the Medical Education office at Toronto General or Toronto Western Hospital. Residents must pay a $50 refundable deposit before receiving their pager.

Residents are responsible for the misuse, loss, or non-return of the pager. The $50 deposit will be forfeited if the pager needs to be replaced. Pagers must be returned within 30 days of the end of the rotation. Failure to do so will result in cancellation of the pager and the loss of the $50 deposit.

Note: Clinical Fellows are issued pagers directly through their department, and not through the Medical Education office.

Travel Reimbursements

UHN Travel Reimbursement Form

In accordance with the 2008 PAIRO Collective Agreement, as stated in Article 19.7 (b), “the Hospital will pay taxi charges to a maximum of $70.00 per month on presentation or appropriate receipts.”

This is in circumstances where:

  • a trainee is on Out of Hospital Call and can respond within the hospital’s Medical Advisory Committee (MAC) approved rules and regulations regarding specified response time,
  • the trainee does not have a parking pass, and
  • The requirement to attend for clinical duties occurs after 6 pm and before 6 am.

3 items are required for processing:

  • – A completed Travel Reimbursement form must be submitted to the Medical Education office.
  • Original receipts of taxi charges are required – these cannot be photocopies,
  • A copy of the call schedule is also required for reimbursement. Call stipend forms are not acceptable.
  • – Claims for reimbursement must be submitted no later than 30 days after the expense was incurred.

Vacation Requests

At Mount Sinai Hospital and the University Health Network, vacation requests are done departmentally. Contact the administrative person for the department in which you will be rotating when you wish to request vacation.

All vacation requests must be submitted at least 30 days before the proposed vacation dates and may be made at any time throughout the year, even after March 1st, EXCEPT where vacation time is being taken to write a certification exam, which may be requested at any time including within the 30-day period before the exam.

VPN Remote Access

Instructions for How to Apply for VPN
Trainees can request VPN Remote Access can by completing a Network Access Request Form (NARF)*. You must complete the NARF exactly as indicated in the instructions.
*The NARF is only available on the UHN Corporate Intranet.  

Dictation

Apply online for your dictation account after receiving your EPR login and password.

Complete the Dictation Access Form in its entirety, and submit. You will be granted access in approximately 2 days (during a normal work week).

Bicycle Parking

Space is available on a first come first serve basis.

UHN bicycle parking facilities are for short term parking for UHN staff only. Long term storage of bicycles is not permitted.

Toronto General Hospital – Indoor bicycle parking is located in the Munk Building parking garage. The Bike Cage has space for about 65 bicycles. A $20 deposit is required to obtain a key to the caged bicycle area. To obtain a key please:

  1. go to the office in the parkade
  2. show your hospital ID
  3. pay the deposit and fill out a form

You may lock up your bike with your own lock in the caged area.

Toronto Western Hospital – Indoor bicycle parking is located in the East Basement Bike Room. Outdoor bicycle parking is located at the Leonard Street Parkade next to the coastal for the East Wing main hospital entrance. A $20 deposit is required to obtain a key that will access both bike and parking facilities. To obtain a key please:

  1. go to the office in the parkade
  2. show your hospital ID
  3. pay the deposit and fill out a form
  4. directions and map to the indoor bike room will be provided to all key holders.

You may lock up your bike with your own lock in the caged area.
