Toronto General and Toronto Western Hospitals

There are call rooms in the hospital. Check with your department for call room locations and availability. Keys and/or combination codes are distributed by individual departments.

Mount Sinai Hospital

See the administrator of your assigned rotation or the Chief Resident for call room locations and entry codes.

The University Health Network and Mount Sinai Hospital assume no responsibility for articles lost or stolen from lockers and/or call rooms. Make sure all valuables are securely stored.

Commissioner for Taking Affidavits (UHN) and Commissioner of Oaths (MSH)

University Health Network

The Commissioner for Taking Affidavits at UHN Medical Education is Samara Gravesande. By appointment only. Contact Samara Gravesande at 416.340.4800 x3265 .

Signing will take place at the Toronto General Hospital Medical Education Office, Eaton Wing, Ground Floor, Room 001.

The Commissioner for Taking Affidavits can and will only sign documents that are related to your education.

Requirements:

  • You must present a valid Wightman-Berris Photo ID Badge.
  • Documents must be filled out, but not signed (these must be signed in the presence of the Commissioner and all photos and copies attached).

Mount Sinai Hospital

The Commissioner of Oaths at Mount Sinai Hospital is Dr. Maureen Shandling (Senior Vice-President, Medical). By Appointment only. Contact Dr. Shandling at 416-586-4800 ext 8848

The Commissioner of Oaths can and will only sign documents that are related to your education.

Requirements:

  • You must present a valid Wightman-Berris Photo ID Badge.
  • Documents must be filled out, but not signed (these must be signed in the presence of the Commissioner and all photos and copies attached).

DICTATION and CliP at UHN

Apply online for your dictation account after receiving your t-ID and EPR login and password.

To gain access to the UHN Dictation System site (on UHN intranet), go to: http://intranet.uhn.ca/clinical_tools/dictation/

Complete the Dictation Access Form in its entirety, and submit. You will be granted access within 2 days (during a normal work week).

For technical problems, call the HELP desk at ext. 14-4357 or Transcription Services at 16-6000.

UHN is now using CliP for self-editing transcribed notes and can be accessed through EPR. There is an optional elearning module for CliP on the Transcription Services webpage – http://intranet.uhn.ca/clinical_tools/dictation/. The elearning module can be found under the “CliP UHN Education” section of the webpage.

Mount Sinai Hospital

Health Records is open 24 hours, seven days a week. Hours of operation for the Transcription Centre, health Records for assigning Dictation ID access codes from Monday to Friday, 7:30 am to 3:30 pm.

EPR AT UHN

Clerks are required to be trained and have access to EPR for all rotations at the University Health Network.

Training and testing for this patient records program will be completed during Transition to Clerkship.

LINENS

Toronto General and Toronto Western Hospitals

Trainees are required to wear scrubs unique to the hospital where they are rotating. Please ensure you have the appropriate scrub sets.

The Uniform Control Form is available at the Medical Education Office or available for download here. Trainees must pay a deposit, refundable once scrubs or lab coats are returned at the end of the clinical placement.

The deposits required for uniforms at the University Health Network are as follows:

  • $25 for 1 scrub set
  • $30 for 1 lab coat
  • $55 for 1 lab coat and 1 scrub set
  • $80 for 1 lab coat and 2 scrub sets

TGH – new issues only – Eaton South Basement, Rm. 408 – M-F, 830am-12pm & 1-4pm

TGH – clean uniform exchange – Clinical Services Building, 3C-438A – M-F, 630am-4pm

TWH – new issues and exchange – MC1-412, M-F – 805am-230pm

PMH – new issues and exchange – 3B-114, M-F – 10am-12pm & 1pm-4pm

NOTE: Deposit will be refunded using same method of deposit payment i.e. cash deposit receives cash refund, credit card deposit receives credit card refund, debit card payment receives debit card refund.

Mount Sinai Hospital

Pick up a Scrub suit form from the MSH MedEd Office, 60 Murray Street, Level 3-300.

There is an $80 refundable deposit that will be collected for 2 pairs of Scrub suits.

Lab coats and scrubs are available at Linen Services on the 1st Level, Rm. 152, M-F – 1030-1130am & 2-3pm

Weekends and holidays CLOSED

LOCKERS

Pre-Clerks will be assigned a locker at one of the three Wightman-Berris Academy sites (Toronto General, Toronto Western or Mount Sinai Hospitals) for a two year period (first and second year).

Clerks are assigned lockers at all three sites for two years (third and fourth year).

Locker assignments can be found on MedSIS.

While on duty, clerks are responsible for the security of their personal belongings.

The University Health Network and Mount Sinai Hospital assumes no responsibility for articles lost or stolen from lockers and/or call rooms. Make sure all valuables are safely and securely stored. 

 At the end of the two period (end of preclerkship OR end of clerkship), please empty all contents from your locker. Any contents remaining at the end of this period will be disposed of.

MAILBOXES

Mailboxes are located at all three hospital sites. NOTE: there is a possibility students may receive mail at any of the three Academy sites.

Toronto Western Hospital – MedEd West, MedWest Building, 750 Dundas Street West, 3rd Flr., Rm. 305

Mount Sinai Hospital – Cooper Centre, 60 Murray Street, 3rd Floor

POWERCHART AT MOUNT SINAI HOSPITAL

Clerks are required to be trained and have access to Powerchart for all rotations at Mount Sinai Hospital.

Training for this patient records program will be completed during Transition to Clerkship.
